El Djem doesn't have much to offer except for the amphitheatre, but the amphitheatre in itself is well worth a visit and making the journey (be it either on a daytrip or as a stop on a longer excursion). It's truly beautiful and very photogenic (taking photos costs 1 dinar extra per camera). Many places are accesible to walk in and take pictures, a lot of the theatre is open to the public. Went on a working day June morning, was not busy at all, did not queue. Toilets were clean. Not suitable for wheelchairs.
